Methods of accounting variation of AKI incident rates among hospitals

A study from Spiegelhalter has shown three methods to account for variations among institutes; a good method to account for the variation enables us to identify extreme outliers and focus our efforts on improvement in targeted areas. This study applies the three methods to rates of Acute Kidney Injury (AKI) in the UK, which vary significantly between hospitals. It describes each method and discusses the benefits of each method. 1. Method 1 - Using an interval as a target 2. Method 2 - Over-dispersion 3. Method 3 - Random effect Comparing the merits of different methods of accounting for variations of AKI rates will help us determine which method is the best in this specific scenario. Identifying extreme outliers of hospitals/trusts can help explain the reasons why trusts have extremely high or low AKI rates compared to other trusts, to allow quality improvement programmes to be targeted.
