The project will evaluate the effectiveness of the Integrated Business Case intervention in Hillingdon in reducing hospital admissions and improving clinical outcomes in patients with chronic conditions. The project will use a quasi-experimental design to follow patients over a period of one year. Examples of outcomes include QA score, length of stay at hospital, and clinical outcomes such as HbA1c. Monthly records will be created for patients with long term conditions. The records will include clinical indicators and unplanned secondary care activity. Example data specifications are included with this application (Three Dataset examples.xlsx). The data will be analysed using interrupted time series analysis and other methods to evaluate the effectiveness of interventions in relation to unplanned hospital admissions, risk factors for admissions for COVID-19 and the effectiveness of telemedicine.
