This project will aim to improve outcomes from services as well as the overall experience of services and service pathways in relation to public health and prevention. Patients may experience improvement in health outcomes and overall service quality.

Public benefit statement

The purpose of this project is to improve population health management in Hounslow. The aim is to help provide relevant, up to date population health management information and surveillance to support and help guide wider public health programmes that involve ICP partners including the CCG, Council and wider partners to ultimately help improve the population’s health and wellbeing in Hounslow. This includes projects such as: - Integrated Care Partnership Public Health and Prevention workstream with specific projects on Type 2 Diabetes. Respiratory task force focusing on COPD, Asthma, smoking and air pollution. Cardiovascular task force. Alcohol work stream. 0-4 early years. - COVID-19 vaccination uptake and analysis - Obesity whole systems approach - Dementia workstream - Frailty and Falls - COVID-19 response and monitoring the epidemiological trends. - Flu Jab uptake - Mortality reports - Excess deaths To use WSIC information as a key evidence base for working towards embedding health wider across the council such as planning, housing, transport and environment teams as part of a ‘Health in All Policies approach.’ In order for Public Health to contribute to wider discussions, projects and workstreams, relevant and accurate health information for the borough at granular levels is required to provide that strong evidence base to support or challenge projects. Additionally, having access to WSIC will help contribute to the ongoing development of the Hounslow Joint Strategic Needs Assessment (JSNA).
