Lay summary

Findings will be used to inform stakeholders of the health of the boroughs, evidence areas for interventions/targeting and for modelling the need for residential care and public health services.

Public benefit statement

We require access to the De-Identified data to describe the health of older people resident or accessing care in Kensington and Chelsea and Westminster and understand the factors that influence their need for residential care and (public) health services. According to the literature one in two care home residents are admitted direct from hospital and the main reason for moving to a care home are mental and physical health conditions (in addition to factors such as living situation, social ties, and financial situation). Population ageing and changing patterns in lifestyle, health, mental health, and care preferences may change future need. We would like to explore these patterns and current service use in our boroughs. This will help understand and model the need for residential care and health services to inform service design and evidence areas for intervention/targeting. Analysis may include: • Describing the prevalence and co-occurrence of long-term conditions recorded on the GP patient registers (including for example dementia, cardiovascular disease, cancer, mental health conditions and obesity) and the Frailty Index in the older population, by patient characteristics such as age, gender, place of residence and if they currently live in a care home or in the community. • Understanding local patterns of admission from hospital to care home • Inform the type of care needed using the social care dataset • Understanding the current lifestyle, general health, mental health, prescribing and health seeking behaviour in the middle-aged (e.g. 45-55 years old) and older population and how this may affect their need for residential care and services in the future.
