West London NHS Trust is undertaking targeted data science and analytics review to enable development of a holistic community based offer for older adults aged 70 years and above with mild/moderate or severe mental illness.

The development of a holistic community based offer for older adults will enable a more joined up provision that increases quality of care, reduces health inequalities and enables resources to be effectively deployed whilst delivering personalised treatment and support for older adults with mental health needs in the community. The development of this offer will bridge the gaps between service provision for older adults and enhance interfaces between primary care networks, GPs, social services, voluntary sector partners and other organisations. In addition, this offer will incorporate physical health care components as well as other specialised considerations for the provision of mental health care for older adults, including links to frailty services This work is underpinned by objectives set out in the NHS Long Term Plan and Community Mental Health framework.
