The aim of T2DEx is to assess the feasibility, usability, acceptability, cost-effectiveness and safety of a remote care service (“Fresh Start”) in primary care for people with ‘high risk’ or ‘very high risk’ Type 2 Diabetes in North West London. The study will assess patient and clinician experience metrics, the feasibility of implementation including uptake and patient/physician acceptability for data collection procedures, and the resources needed to manage and implement Fresh Start . The study will also assess the impact of Fresh Start on clinical outcome measures (e.g., HbA1c, risk factor control, time at goal). The results of this may support continuation of Fresh Start.
