The aim of this study is to undertake a health economic analysis of the health and social care service use of patients with the most severe and complex relational disturbance. The term relational disturbance refers to the broad range of physical and mental health diagnoses which evidence indicates arise from attachment disorders. Previous studies have identified high service use and health economic costs of providing services for people with relational disturbance by comparison with other mental health problems. It is recognised that a small group of highly complex patients with personality disorder and related diagnoses undergo extended admissions to physical and mental health inpatient settings. The costs of this highly complex group are estimated to be in the region of £40M per 1 million population. The pattern of service use and associated costs for outliers at the severe end of the spectrum of this disorder have not been studied. There is evidence that effective psychotherapeutic interventions improve clinical outcomes and reduce costs for this patient group. This study aims to measure the costs of this most complex patient group across health and social care to make the case for development of psychotherapeutic services. This study addresses two questions: 1) What are the costs and patterns of health and social care use by patients under the care of Devon Partnership Trust and West London NHS Trust with the most severe relational disturbance? 2) What are the clinical and service use predictors which would facilitate identification of these patients within public sector services to establish service need and resource allocation to therapeutic pathways?
