Our project is specifically looking at health inequalities for patients in NWL with AF, a cardiac condition. The NICE approved innovation is the use of DOACs – to be more effective for the prevention of AF-related strokes than other anticoagulants.
Public benefit statement
The overarching aim of this project is to take a population health management approach to reducing inequalities in access, diagnosis and treatment of patients with AF. We will do this by: 1. Highlighting pockets of inequalities and identifying unmet needs 2. Prevention: proactive identification and evidence-based management of AF 3. Patient and public engagement: targeted engagement with identified communities with greatest unmet needs, increasing our understanding of the barriers and possible solutions. Specifically, this may lead us to engaging with patient groups in North West London who may be less likely to be diagnosed with Atrial Fibrillation, be prescribed DOACs, or continue their prescription of DOACs. Page 4 of 22 This will be with the aim of increasing their access to appropriate diagnosis, and treatment using DOACs where possible, which will in turn reduce their risk of AF-related strokes.
