The purpose of this project is to evaluate the effectiveness of CLCH services (where possible) using linked datasets in WSIC, with a view to service improvement.
Public benefit statement
CLCH provides a range of services to children and adults in the NW London area. Whilst CLCH are able to monitor activity levels and some limited clinical outcome data for the patients they see, many of the measures of success sit within other services. For example: - Control of key clinical measures for patients with diabetes after referral to the CLCH community diabetes service (captured in GP practice data) - Hospital admission avoidance following enhanced district nursing support (hospital inpatient and A&E data) WSIC offers the possibility to evaluate the success of some CLCH services in NW London against these outcome measures, first on a one-off investigative basis and potentially later on a routine basis. The purpose of this project is therefore to evaluate the effectiveness of CLCH services (where possible) using linked datasets in WSIC, with a view to service improvement.
