Public benefit statement
This current project will not benefit patients directly; however, it forms the basis of our future work to develop a mechanism that can identify and flag to hospital clinicians patients’ individual falls risk factors from their GP records. We have conducted patient and public involvement activities with two groups of older patients and their family members/informal carers: they told us that they are frustrated at being repeatedly asked the same questions by multiple professionals in different organisations. They said joined-up care is important to them and they understand this requires responsible information-sharing between healthcare providers. A future Clinical Decision Support tool to surface important falls risk factors from GP records to hospital clinicians has the potential to benefit patients by providing clinicians with key information to enable targeted falls risk interventions for individual hospital inpatients. This could lead to a reduction in harm from falls in the hospital setting. This data-driven overview of a falls risk factors will provide valuable insights into why specific patient groups fall; this could lead to targeted interventions to reduce health inequalities across the North-West London population.
