We are keen to undertake a service evaluation of the Know Diabetes service in order to analyse patient outcomes and the cost effectiveness of the platform.

Public benefit statement

There are approx. 170,000 people living with diabetes and 210,000 with non-diabetic hyperglycaemia in North West London. Type 2 diabetes in particular is associated with higher levels of deprivation and BAME ethnicity. There are clearly demonstrable benefits from similar diabetes platforms in other parts of the country. In order to determine whether to continue funding Know Diabetes beyond this year, we wish to analyse the improvement in clinical outcomes and resource utilisation which are associated with the platform.We are keen to undertake a service evaluation of the Know Diabetes service in order to analyse patient outcomes and the cost effectiveness of the platform. The key components of the analysis will include: clinical parameters (e.g. HbA1c, blood pressure, etc), health resource utilization (including GP and acute hospital activity), diagnoses (e.g. heart attacks or strokes). We will also analyse deprivation and ethnicity data in particular in order to look at the impact on health inequalities.
