The purpose of these missions are to develop a a more coordinated, collaborative approach that has the potential to increase the impact for the population in North West London. Mission 1 By evaluating the current services, and answering some of the research questions, we hope to maximise the experience and outcomes for residents with chronic cardiovascular disease by preventing, diagnosing and treating the persons needs. The impact of these projects would have impactful patient benefits such as finding undiagnosed cases, understanding hesitancy in various populations to come forward/comply with treatment interventions, and development more/better ways of improving adherence to interventions that free up capacity in primary care. Mission 2 Sadly our residents are sicker and turning up to our acute hospitals with more complex needs which is creating a system under significant strain. By evaluating current emergency care pathways, inpatient stays and discharge flow, we will create impactful research questions that will minimise harm suffered by patients being in the wrong care setting with the aim of seeing people in the right place at the right time, every time. We need to understand the underlying drivers for demand, the resources available to support people outside the acute system, and the opportunities that arise from technological advancements. Mission 3 By evaluating the current services, and answering some of the research questions, we aim to take action to ensure that children and young adults have the best start in life, with a particular focus on promoting positive mental health and enabling an appropriate range to support to enable early and effective intervention. The targeted outcomes of the Mission work from ICHP will provide tangible, beneficial opportunities to inform, change, and transform the healthcare system in NWL – at the benefit of NWL patients.
