ntermediate care services provide support to individuals for a short time to help you recover and increase your independence. This is usually following a stay at hospital and can take place at home, in a care home or in a hospital. This research will help us understand what makes intermediate care successful, where we have gaps in our services and which patient groups benefit the most. It will also help us understand which patients are not accessing intermediate care services when they need to. We hope that the findings of this will help improve the intermediate care services that NWL NHS system provides. Throughout this project, we are working directly with 8 residents of NWL with experience of intermediate care, who have and will continue to shape our approach and help us interpret our findings.
