We are looking at trying to determine whether there are any predictive factors that could help us take steps to help prevent the circumstances leading to hospital stays and reablement becoming necessary.
Public benefit statement
We would ultimately use the insights gained from the analysis to improve the patients’ experience of care and the quality of the services by: • Reducing unnecessary hospital admissions • Streamlining services, and removing any duplication • Making the transfers of care seamless • Designing care to be wrapped around the individual rather than a one-size-fits-all approach • Reducing the need for long-term care where unnecessary We would also improve service sustainability by informing the development of a model that is financially viable and offers value to the system.
