Public benefit statement
The purpose of this project is to test and evaluate how the standard model of care for HIUs can be developed to fit the needs of people resident in North-West London. It will explore how best to design and organise care including how to collate and share information from multiple providers, coordinate care between different agencies, and provide direct support for some people with more complex needs. The intended outcome of this project will be to improve patient experience and benefit by exposure to fewer harmful procedures, improved mental and physical health outcomes and satisfaction with the health system, increased knowledge about services, and improved quality of life and wellbeing. Further intended outcomes include developing a cross-border service, change in health usage and value for money, and improved staff experience. All of which, will indirectly benefit patient care.
