HFpEF @ Home is a nurse led project to be implemented by the cardiology services in West Middlesex University Hospital.

Public benefit statement

HFpEF @ Home will provide tailored care for patients living with Heart failure with Preserved Ejection Fraction (HFpEF) through a dedicated remote monitoring hub. The aim is to improve HFpEF patients’ experience of care, to reduce variability in the quality of care received and to provide improved education and support enabling patients to manage their condition more effectively and confidently at home. We also aim to reduce the number of readmissions to hospital for this cohort of patients. This project will evaluate this new care pathway at West Middlesex University Hospital for patients with HFpEF, using patient surveys and focus groups, as well as data linkage and analysis of healthcare usage. We believe that this project will directly impact on the patient, staff, service level/ organisation and the environment. Through this intervention, we hope to achieve the following: • Improved self-care management. • Improved patient and staff experience. • Deliver long term health benefits to patients. • Resource saving for the Trust.
