The purpose of this research project is to investigate the potential of Machine Learning to predict patient outcomes for Population Health Management.

Modelling future patient needs and health inequalities has the potential to: - Help ensure that the right services can be commissioned ahead of time to meet the needs of NWL patients Version 4.0 – April 2023 Page 4 of 23 - Provide predictive analytics to support the reduction of health inequalities in NWL - Enable targeted preventative care interventions to support patients to remain healthier for longer The outcomes of this project will be published in a peer-reviewed journal so that new insights can contribute to patient care. Any models that are developed as part of this project will be used to benefit patients in North West London. We are having discussions with the NWL Population Health and Health Inequalities team to ensure that the outcomes of this project are aligned to their needs and priorities. We will continue this through the duration of the project. This project will target COPD / Emergency admissions as a first step, but the intention is that the methodological approach can be transferred to other Long Term Conditions / health outcomes if successful.
