This audit aims to explore a number of questions to help ensure that patients receive the right care, at the right time, in the right place.

Public benefit statement

The audit will help us as a system to identify gaps in providing safe and appropriate care for residents within their care setting therefore improving the long-term quality of care. The audit will help to inform better understanding of: • The reasons for A&E attendances from care home residents • The care provision before and after A&E attendances • Good practice and areas for improving care delivery to support residents within care homes. This learning will be shared across all care homes and care home GPs The audit will involve assessing care home residents who had A&E attendances to hospital but were not admitted to establish: 1. What was the time of attendance? 2. What was the reason for the attendance? 3. Is a care plan in place for the resident? 4. Was there a care plan in place prior to attendance? 5. What type of care plan (for example: My Care My Way, North West London Digital Personalised Care and Support Plan) was created for the resident? 6. Does the resident have a Universal Care Plan in place? 7. Was the care plan/Universal Care Plan accessed prior to the attendance? 8. If there was a previous admission for the resident, was there a post-admission review of the care plan within seven days 9. Is there any learning from the created care plan i.e. quality, process etc.?
