The aim was to Introduce ‘Making Every Contact Count’ approach in the pre-assessment unit of engaging in conversations with patients about their lifestyle and providing the tools and information they need to make meaningful changes

Public benefit statement

1. Short Term benefit: -We expect the surgery call back time will be reduced. The reduction in time to call back a patient for surgery will be measured. -Cost effectiveness impact for the Trust: Reduction in call back time for surgery will enable to treat more patients during the year. 2.Long Term benefit: -No of patients stopped smoking & drinking alcohol( or reduced drinking) -Reduced reading of blood pressure & blood sugar -Number of patients practicing healthy eating & exercise -Number of patients improved medication adherence -Reduced BMI -Cost effectiveness impact for the Trust: PHM intervention will reduce number of patients attending GP, A & E and admission to hospital by promoting and supporting healthy living.
