HFpEF @ Home is a nurse led project to be implemented by the cardiology services in West Middlesex University Hospital.
Public benefit statement
The aim is to improve HFpEF patients’ experience of care, to reduce variability in the quality of care received and to provide improved education and support enabling patients to manage their condition more effectively and confidently at home. We also aim to reduce the number of readmissions to hospital for this cohort of patients.This project will evaluate this new care pathway at West Middlesex University Hospital for patients with HFpEF, using patient surveys and focus groups, as well as data linkage and analysis of healthcare usage.
