This study evaluates clinical phenotypes of preterm birth and related risk factors to guide prediction and treatment. It also examines risks of recurrence in women with prior preterm births, aiming to improve care based on phenotype-specific profiles.

Public benefit statement

In the UK, approximately 8% of all births are preterm (before 37 completed weeks of pregnancy). Preterm birth is the single largest cause of death in babies and children under the age of five years, and many babies who survive go on to have long-term health problems or disability. There are multiple different causes of preterm birth, including those women who go into labour spontaneously, and those births which are provider-initiated due to concern about the mother or baby. Current prediction of preterm birth is poor, especially for women in their first pregnancy. Many women having their first preterm birth have none of the currently identified clinical risk factors. These data allow consideration of multiple risk factors including socio-economic risk factors such as level of deprivation and exposure to air pollution.
