The purpose of this project is to develop and apply a neighbourhood-level population health and system impact modelling approach to support the design, implementation and evaluation of Integrated Neighbourhood Teams (INTs) and the wider Neighbourhood Health Service across North West London. The project aims to provide commissioners and place-based partnerships with a robust, shared evidence base to inform strategic commissioning, investment decisions and delivery planning as care is shifted towards more proactive, preventative and integrated neighbourhood models.

The outcomes of this project will benefit patients by helping the health and care system deliver more joined up, proactive and equitable care at neighbourhood level.
