END STAGE KIDNEY DISEASE FROM GLOMERULONEPHRITIS IN THE UK – A REGISTRY BASED STUDY (GN-UNITY)

Glomerulonephritis (GN) is a disease which affects the tiny filters inside the kidneys. It is one of the commonest causes of End Stage Kidney Disease (ESKD) across the world and is associated with significant illness, mortality & healthcare burden. Patients with GN have a 2.7-3.9 fold rise in death rate compared with the rest of the population. The three main forms of GN that could lead to ESKD include IgA Nephropathy (IgAN), Membranous Nephropathy (MN) and Focal Segmental Glomerulosclerosis (FSGS). The prevalence of the individual subtypes of GN is poorly understood and their impact on incidence and prevalence of ESKD and subsequent mortality is largely unknown. The incidence of patients with GN who have progressed to ESKD, and their outcomes on the various types of renal replacement therapy (RRT) and rates of death has previously not been quantified. This study will evaluate outcomes after ESKD and characteristics of RRT in the three most common Primary Glomerular Diseases that lead to ESKD. We will use UK Renal Registry data to identify all patients who started RRT with GN as a primary disease during the years 2009-2018. We will use Polycystic Kidney Disease (PKD) as a comparator to help analyse the outcomes and draw differences. About 12-13,000 patients in total would be included in the study. Outlining trends in incidence and prevalence of ESKD in different GNs would help focus efforts on making improvements to patient care. This study will also provide better information to patients to assess choice of modalities of RRT for GN. The financial burden associated with ESKD in these groups of diseases will be analysed from a health service perspective.
