Patient pathway description

The patient pathway in primary care begins with registration at a GP practice, where core demographic information and any known clinical history are recorded. During each contact, GPs and other practice staff record coded entries for symptoms, diagnoses, prescriptions, referrals, and results. GP data therefore provides a continuous, foundational record of each patient’s interaction with the NHS.
