This dashboard will provide clinicians and commissioners with population level data on diabetes and related foot issues in the population registered with around 400 General Practices in NW London. The dashboard will also allow a ‘radar’ view (patient level) for clinicians to support better care planning and management of patients with a view to reduce unscheduled hospital admissions for diabetes foot disease as well as unwanted amputations. Clinical uses of the Foot Care Dashboard: • To provide a suite of information dashboards for use by clinicians and commissioners for managing care and commissioning services based on the population need. • To visualise the prevalence of foot care problems and outcomes within the diabetes population, and to understand the population at risk. • To monitor trends over time for specific indicators. • To enable understanding of pathways and the impacts they have on the patients’ outcome.
