The Frailty Population Health dashboard will build information and create intelligence about frail patients, through aggregate data analysis from multiple sources. The dashboard will be accessible by a range of professionals, for different purposes, Front line clinicians will have access to patient-identifiable information for patients they are caring for, Operational managers and system leaders will have access to population health data (nonpatient- identifiable) depending on their location, to inform strategic decision making and service-level quality improvement and management.
