The High Intensity Use (HIU) dashboard will offer an opportunity to ascertain individual cases that are more likely to be amenable to timely intervention. This means that clinicians can obtain a list of patient information on a regular basis for their HIU cohort based on whole system usage. As part of this, a High Intensity Use service will be developed in each borough and Acute, Primary Care, and Community colleagues, will work in partnership to facilitate access to information and improve patient outcomes. The suite of tools will also aid Healthcare Professionals to, Support proactive case finding, Improve quality of care as clinicians can target patients proactively, Create time for care professionals to care for patients, Most importantly there is a direct effect on workforce development in utilising digital tools in patient care.
