LADS Patient Cohort Dashboard

Description

The aims of the Asthma LADS Tool are: To improve health outcomes and care for asthma patients across London by supporting population health management and delivery of the NHS long term plan for asthma patients of all ages, Identifying and reducing health inequalities, Enabling data driven healthcare (care where needed) and successful asthma quality improvement programmes, Supporting decision making at all levels by combining primary care, hospital care, medication usage, air pollution exposure and demographic information into one tool, To be an exemplar project that brings together NWL and SEL ICS around a single scalable, clinically actionable dataset, To complement existing sources of data extraction in bite-sized or more detailed chunks. e.g. commonly used by PCNs - open prescribing, prescripp, Ardens, EMIS, Systems one etc. Please refer to the dashboard user guide – https://www.nwlondonicb.nhs.uk/download_file/view/3221/1927

Results/Insights

The LADS tool has several functions to support improving asthma care in London: • Highlight areas of need across NWL and SEL, at borough, PCN and practice level • Support decision making at all levels by combining primary care, hospital care, medication usage, air pollution exposure and demographic information into one tool • Enable a population health management approach to improvement, using the triangulation of data as described above. This will enable improvement projects to be set up to meet the needs of the local populations • Ensure resources are used as efficiently and effectively as possible and specialist care directed to need • Used to monitor the outcomes from improvement projects in asthma care
